Milsestone - Based Research Partnership Form

This form is for researchers or organizations who have agreed to partner with Invisible Warriors on a milestone-based research project. Invisible Warriors is funding this project and will define the research scope, milestones, and required data.

The purpose of this research project is to understand real Congenital Heart Defect (CHD) issues as experienced by families, not just what large corporations report. This is intended to be an uncontrolled research project, fully directed and funded by Invisible Warriors. Funding for this project will continue only until the agreed-upon milestones are completed. A new research project may be initiated afterward at Invisible Warriors’ discretion.

I confirm that I am authorized to submit this information and enter into this research partnership. I understand that this is a milestone-based project funded and directed by Invisible Warriors. Invisible Warriors may pause or terminate funding at any time if:

• The research is not meeting the objectives outlined,

• The required data is not being provided, or

• Any milestones are not completed as agreed.

I understand that funding for this project will end once the agreed-upon milestones are completed, and that a new research project may be offered afterward at Invisible Warriors’ discretion.

I agree to follow the research scope, milestones, and instructions as outlined by Invisible Warriors. I release and hold harmless Invisible Warriors from any claims, liabilities, or damages arising from this project.
