Family Research Submission Form

This form is for families who want to share their child’s Congenital Heart Defect (CHD) experiences to help Invisible Warriors better understand real-world challenges, gaps in care, and outcomes.

Submissions are used for research, analysis, and awareness. Participation is voluntary, and providing information does not guarantee public sharing or program placement. Families may be contacted for follow-up questions, but all submitted information will be handled respectfully and securely.

I confirm that I am the parent or legal guardian of the child listed, or otherwise legally authorized to provide this information and any accompanying medical records.

I confirm that I have the legal right to submit all information and documentation provided and that it is accurate to the best of my knowledge.

I understand that this submission is intended for research and data-gathering purposes only and does not constitute medical advice, diagnosis, treatment, or clinical care.

I understand that Invisible Warriors supports uncontrolled, exploratory research focused on real-world Congenital Heart Defect (CHD) experiences, including delayed diagnosis, outcomes, and care gaps.

I understand that my submission may be reviewed, analyzed, and shared with research partners in a de-identified and aggregated form.

I understand that participation is voluntary, that I will not receive compensation, and that I may request withdrawal of my submission from future research use by contacting Invisible Warriors. Data already analyzed or published in de-identified form may not be retractable.

I release and hold harmless Invisible Warriors, its representatives, and research partners from any claims, liabilities, or damages arising from the use of this information for the purposes described above.
